From the time the kids were little, I would conduct this exact survey with them each year.  I got this from the J2Ks that have been in our lives for so long.  (J2K is the birth board family that I've been a part of since 11/1999 when I found out I was expecting Ryan.  J2K stands for July 2000)

What is your favorite thing to do? video games, watch TV
What is your favorite piece of clothing?  (mommy laughing because she knows Ry dislikes pants)  ???
What was your Halloween costume this year?  Red Angry Bird
What is your favorite animal? puppy - i REALLY want a puppy
What is your favorite holiday? Christmas
Where is your favorite place to go?  Hawaii or Tahoe
What is your favorite room in your house?  my room
What is your favorite food?  beef teriyaki from Kyoku,  ribs, spaghetti with a LOT of parmesan
What is your least favorite food?  vegetables!
What is your favorite snack?  Cup o' Noodles
What do you want to be when you grow up?   I want to design video games, or design buildings, or cook

Who's your best friend?   My brothers from another mother --- Jerrod, Joshua &  Tyler, I also like to hang out with Alessandro, and miss my friend, Xavier.
Who is your favorite person to hug?  my mom - because she's typing this.
Do you have any Pets?  fish - Crystal and friends
Do you have dolls and which is your favorite?   um... no.  i do have some stuffed animals and lots of figures
Do you sit still to get your hair brushed ?  (laughs) um... i don't have to brush my hair.  it's too short
What does your room look like?  right now, it's clean.  haha!  my room is green.  bed has either Star Wars, Clone Wars or Sports bedding.
Do you like school?   yes.
What is your teacher's name?   Mr. H is my main teacher.  I also have Mr. Sasser for Math/PE and Mrs. C for Science.
Do you like to go shopping?   for myself, yes.
Do you like to go to the grocery store?   not so much
Do you like having your picture taken?  yes.  Mommy takes my picture too much, though.
Do you like to sing and dance?  sometimes
Would you rather have pepsi or milk?   neither.  i like Coke... or chocolate milk.  i REALLY like vanilla milkshakes with rainbow sprinkles.
What's your favorite ice cream or cupcakes?  mint chocolate chip ice cream.  yellow cupcakes.
How far can you count?   as high as i need to.
Can you ride a bike? yes.
Do you like to swim? yes.
What makes you feel very happy?  not having to do work around the house... or homework.
Very sad?   haha!  having to do homework and work around the house.
Very angry?  i don't really get angry.
